<International Circulation>：Will clinical practice towards treating MR change in the near future?

　　Dr. Zoghbi: There are investigations to see if there are any medical treatments， particularly for functional regurgitation. Can pharmacotherapy can really improve prognosis? Number two， further refinement in repair technology. You have seen as of late that more and more repair technology that spares the mitral valve and addresses its mechanism by having an anularplasty in addition to artificial chordae as opposed to the resection of part of the valve. We will see more of that as it preserves some of the functionality of the annulus and also some of the functionality of the mitral valve apparatus. You see more and more refinement of that hopefully for better results acutely and in the long term.

　　Zoghbi博士：有关于药物治疗的研究，尤其是对于功能性反流。药物治疗是否能能改善预后呢？关于修复技术的进一步改进方面，二尖瓣修复技术近来也越来越多，其机制并非为切除部分瓣膜，而是使用人工腱索的瓣环成形术。这些技术会保护瓣环和二尖瓣的功能。技术方面的改进会越来越多，有希望取得近期和远期更好的结果。

　　<International Circulation>：When it comes to medically treating these patients， how do feel about the use of ACE inhibitors?

　　《国际循环》：关于对这部分患者的药物治疗，您对ACEI的使用有何看法？

　　Dr. Zoghbi: The earlier data is not very strong， however there are prospective trials that are ongoing. Hopefully they will help us define the role of medical therapy. But the trend is for more aggressive， earlier intervention to fix the mitral regurgitation. In a way， to preserve the structure of the heart ， less than if you allow massive deformation that could occur down the line and mostly to harness of the advantage of keeping the patient in sinus rhythm and not needing echo ablation if they go into AF.

　　Zoghbi博士：尽管有一些在进行的前瞻性试验，之前的证据还不够强。希望这些试验能够帮助我们明确药物治疗的角色。但是目前的趋势是更强化、更早期的干预来治疗二尖瓣反流。某种程度上来说，在心脏发生结构改变之前保护心脏结构可以使患者保持窦性节律，避免了患者发生房颤而需要接受消融术，从而使患者获益。

　　<International Circulation>：I would like to ask for a quick comment on Chinese participation at the ACC Scientific Sessions. What are you looking forward to with China’s presence?

　　《国际循环》：您如何看待ACC会议的中国参会者？对中国参加ACC会议有哪些期待？

　　Dr. Zogbhi: Certainly we welcome our Chinese participants. They have been supporters of our meeting and we have been supportive of theirs. I was in China in August and October and we look forward to continued interaction and mutual exchange of information. Part of the international luncheon forums will be with our Chinese chapter. We also have a dedicated session for the China contingency. We really look forward to welcoming them throughout Scientific Sessions.
